During the week of June 21, the Jefferson County Health Department inspected 44 food establishments. Scores ranged from 65-100.
The inspection scores for the week of June 21 are listed below. They are all routine inspections unless otherwise noted. Facilities considered low priority are not assigned a score. A low priority facility does not prepare food.
■ Adventures in Learning, 1109 Clarke St., De Soto, scored 95 on June 22. Violations: black residue on can opener blade; containers stored on floor; and bags and utensils stored under hand sink with leaking pipe.
■ Connect Church STL, 1779 Springdale Blvd., Fenton, was inspected on June 24. No score due to low priority.
■ Crystal City Pool snack bar, 1400 Dick Cook Drive, Crystal City, scored 99 on June 23. Violation: no thermometer in cooler.
■ Culver’s, 123 Richardson Crossing, Arnold, scored 94 on June 22. Violations: incorrect sanitizer in dish machine.
■ De Soto Head Start, 3225 Baisch Drive, De Soto, scored 90 on June 24. Violations: dented cans; food without use-by dates; incorrect sanitizer testing strips; and soiled fan housing.
■ Dollar General, 102 Twin City Mall, Crystal City, was inspected on June 22. No score due to low priority.
■ Dollar Tree, 732 S. Truman Blvd., Crystal City, was inspected on June 22. No score due to low priority.
■ Domino’s Pizza, 801 Collins Drive, Festus, scored 84 on June 25. Violations: employee put gloves on without washing hands; unlabeled spray bottle; gap in between wall and door frame; dusty vent guards; and peeling paint on wall above dish sink/missing coving.
■ El Lobo Rojo, 106 N. Main St., De Soto, scored 65 on June 25. Violations: only two food safety certificates on site; no staff with manager food safety certification; no illness policy; soiled can opener; several foods without use-by dates; no thermometers in reach-in coolers; bugs in light guards; no covers on foods on steam table; containers stacked wet; panels placed on wire shelving where dishes dry; torn gasket; mineral buildup in dishwasher; wash cycle not reaching 120 degrees; plastic wrap on cooler peeling/soiled gaskets; hot water in hand sink not working correctly; drywall tape on restroom ceiling peeling; and no cover grate on HVAC vent.
■ Festus 8 Cinema, 1522 Parkway West, Festus, scored 92 on June 25. Violations: several staff without food safety certification; ice scoops in disrepair; gaps in doors; rodent droppings in non-food storage area at back of kitchen; soiled food prep cooler bottom, metal drying racks and butter machine cabinets; and water leaking from ceiling in hallway.
■ Festus Manor, 627 Westwood Drive, Festus, scored 88 on June 24. Violations: heavily dented cans; incorrect sanitizer for dish machine; and cooked roast beef being thawed without use-by date marked.
■ Gracelife Chapel kitchen, 1 Victory Drive, Pevely, scored 99 on June 25. Violation: boxes of paper towels stored on floor.
■ Hillsboro Community Civic Center, 10345 Hwy. 21, scored 97 on June 24. Violations: soiled grease filters and ice machine surface; broken tile/exposed electrical socket; and soiled floor in back storage room.
■ Imperial Bowl, 6045 W. Outer Road, Imperial, was reinspected on June 24. No score given.
■ Jimmy John’s, 1300 Veterans Parkway Suite 106, Festus, scored 99 on June 24. Violation: ice scoop stored on cardboard box.
■ Joachim Golf Club, 959 Scenic Drive, was inspected on June 24. No score given due to low priority.
■ La Casona, 5660 Hwy. PP, High Ridge, scored 93 on June 25. Violations: damaged weather strip on back door; utensil held together by tape; cardboard used as lining; ice buildup and broken light bulb in walk-in freezer; and water leak and water damage in ceiling above ice machine in basement.

■ Meadows Club House, 508 Woodland Meadows Drive, Arnold, scored 92 on June 24. Violations: eggs stored above ready-to-eat food; soiled cooler shelves and microwave/dusty cabinet; and women’s restroom door not self-closing.
■ Meadowview Assisted Living, 555 Woodland Villas, Arnold, scored 84 on June 23. Violations: raw eggs stored above ready-to-eat foods; soiled can opener; no sanitizer at dish sink; and missing coving.
■ My Place Residential Care, 23 N. Sixth St., Festus, scored 99 on June 22. Violation: dull can opener blade.
■ Neighborhood Grocery, 306 Bailey Road, Crystal City, scored 100 on June 24.
■ On the Grind Coffee Co., 1277 N. Truman Blvd., Crystal City, scored 75 on June 25. Violations: person in charge does not have manager food safety certificate; some employees without food safety certificates; cooler not keeping food at proper temperature; cooked sausage with incorrect date mark on it; unlabeled spray bottle; boxes of single service items stored on floor; gasket and cooler in disrepair; soiled baking pans; soiled coolers; and broken floor tiles.
■ Papa John’s, 1201 Water Tower Plaza, Arnold, was reinspected on June 25. No score given.
■ Pear Tree Inn, 1201 Drury Lane, Arnold, scored 100 on June 25.
■ Pine Valley at the Woodlands, 620 Woodlands Meadows Drive, Arnold, scored 92 on June 24. ■ Superior Manor of Festus, 12827 Hwy. TT, Festus, scored 77 on June 23. Violations: incorrect sanitizer solution and no sanitizer in dishwasher; hot dogs with no dates marked; sanitizer wipes stored on shelf with cans; torn gasket; pans drying on towels; and buildup of brush around both lagoon cells and vines on fence around second cell of lagoon/duckweed buildup on lagoon cell.
■ Sushi Ai, 2000 Richardson Road Suite 1, Arnold, scored 96 on June 22. Violations: rice cooling improperly; and soiled fryers.
■ Taco Bell, 4698 Hwy. W., House Springs, scored 89 on June 23. Violations: no staff with manager food safety certificate; no thermometer in cooler; gap in doors; and soiled soda machine, cabinet, handles on heat cabinet, top of bins.
■ Tanglefoot Dance Hall, 201 E. Main St., Festus, scored 94 on June 24. Violations: single-serve items stored on floor; no lid to right side of deep freezer; no sanitizer test strips; and leak in dish sink on left side.
■ That’sa Nice’a Pizza, 2000 Richardson Road Suite 2, Arnold, was reinspected on June 25. No score given.
■ U-Gas, 1880 Little Brennan Road, High Ridge, scored 98 on June 24. Violations: burned out light; missing coving.
■ Valle Market, 3245 Hwy. V, De Soto, had a field visit that states “no progress on violations due to staffing issues/reinspection will be pushed back one month to allow more time for compliance.” (Business received a score of 81 on May 26.)
■ Vianney Renewal Center, 6467 Eime Road, Dittmer, scored 96 on June 26. Violations: two staff with expired food safety certificates; and soiled can opener.
■ Waffle House, 3960 Vogel Road, Arnold, was reinspected on June 25. No score given.
■ Wee Care Too Learning Center, 1302 Kenner St., Crystal City, scored 100 on June 25.

How establishments are scored
According to the Jefferson County Health Department, facilities start with a score of 100, and points are reduced based on observed violations:
■ Six points are deducted for priority violations that directly contribute to foodborne illness risk.
■ Three points are deducted for priority foundation violations that indirectly contribute to foodborne illness risk or enable priority violations.
■ One point for core violations related to general sanitation and good retail practices.
■ Two points for repeat violations of any type.
Dan Kane, environmental supervisor for the Health Department, said the agency does not have ranges for good or bad scores but instead looks at the overall score, adding that readers need to look at the types of violations.
He said a couple of priority violations are more concerning than several core violations.
In certain cases, the Health Department returns to a food establishment to reinspect, allowing time for the facility to correct issues. According to the Health Department, reinspection is not solely based on the score an establishment gets; it’s determined by the type of violation. Priority and priority foundation violations need to be corrected within a certain amount of time, and priority violations typically are corrected on the day of the inspection before the inspector leaves.
Kane said the department rarely closes a food establishment, and most temporary closures involve a power outage, fire or sewage backup.
