During the week of March 15, the Jefferson County Health Department inspected 35 food establishments. Scores ranged from 64 to 100. Some were routine inspections while others were reinspections where scores are not assigned. All inspections are unannounced visits.
Facilities considered low priority are not assigned a score. A low priority facility does not prepare food.
According to the Jefferson County Health Department, facilities start with a score of 100, and points are reduced based on observed violations:
■ Six points are deducted for priority violations that directly contribute to foodborne illness risk.
■ Three points are deducted for priority foundation violations that indirectly contribute to foodborne illness risk or enable priority violations.
■ One point for core violations related to general sanitation and good retail practices.
■ Two points for repeat violations of any type.
Dan Kane, environmental supervisor for the Health Department, said the agency does not have ranges for good or bad scores but instead looks at the overall score, adding that readers need to look at the types of violations.
He said a couple of priority violations are more concerning than several core violations.
In certain cases, the Health Department returns to a food establishment to reinspect, allowing time for the facility to correct issues. According to the Health Department, reinspection is not solely based on the score an establishment gets; it’s determined by the type of violation. Priority and priority foundation violations need to be corrected within a certain amount of time, and priority violations typically are corrected on the day of the inspection before the inspector leaves.
Kane said the department rarely closes a food establishment, and most temporary closures involve a power outage, fire or sewage backup.

The inspection scores for the week of March 15 are listed below. They are all routine inspections unless otherwise noted.
■ Arbor View Nursing and Rehab Center, 6400 The Cedars Court, Cedar Hill, scored 94 on March 19. Violations: open cans of soda on cookline; soiled can opener; two walk-in coolers out of order; and sagging ceiling tiles in dry storage with what appears to be water damage and mold.
■ Creative Minds Childcare and Learning Center, 7049 Hwy. BB, Cedar Hill, scored 92 on March 18. Violations: manager’s food safety certificate expired; and thermometer missing in cooler.
■ Crystal Oaks, 1500 Calvary Church Road, Festus, scored 98 on March 16. Violations: ice scoops stored on cloth towel; and gasket in disrepair.
■ Danby Rush Tower Middle School, 1250 Dooling Hollow Road, Festus, scored 100 on March 18.
■ De Soto Early Childhood Center, 1812 Rock Road, De Soto, scored 96 on March 19. Violations: dented can; and soiled microwave.
■ Dollar General, 5697 Old Hwy. 21, House Springs, was inspected on March 17. No score given due to low priority.
■ Dollar General, 7 Walters Plaza, House Springs, was inspected on March 17. No score given due to low priority.
■ Domino’s Pizza, 1951 Richardson Road, Arnold, was reinspected on March 16. No score given.
■ Four Lakes Restaurant and Event Center, 5925 Plantation Drive Suite A, Hillsboro, was reinspected on March 18. No score given.
■ Frankie Gianino’s Grill and Bar, 1209 Main St., Imperial, scored 64 on March 20. Violations: employee with open drink on cookline; employees not using paper towels to turn faucet off; soiled can opener blade and ice machine deflector plate; pizza prep station and cooler drawer not keeping foods at proper temperature; sanitizer bucket stored over bags of flour; reach-in cooler not closing tightly or keeping cool; knives stored improperly; gap between hand sink and wall; debris buildup on shelf in walk-in cooler; hot water in hand sink at incorrect temperature; dish sink leaking heavily from pipe; dumpster missing drain plug; and water-damaged ceiling tiles in restroom.
■ Gas Mart, 2099 Old Hwy. 21, Imperial, scored 97 on March 17. Violation: bag in box sodas stored on floor under shelving unit.
■ Hardee’s, 1305 Veterans Blvd., Festus, scored 82 on March 17. Violations: incorrect sanitizer in dish sink; cooler holding water; debris in cooler/black mold-like film on caulking on dish sink/grease buildup around fryers; leak in hand sink faucet; uncovered trash can in restroom stall; no trash can at hand sink; missing grout; dusty ceiling above reach-in freezer; and soiled hood filters.
■ Jefferson R-7 High School, 7 Blue Jay Way, Festus, scored 94 on March 18. Violation: incorrect sanitizer in dish sink.
■ Jefferson R-7 Preschool, 1265 Dooling Hollow Road, Festus, scored 90 on March 19. Violations: unlabeled spray bottle; missing caulking on hand sink; and clogged hand sink.
■ Kenny’s Bar and Grill, 7045 Hwy. 61-67, Barnhart, scored 79 on March 17. Violations: no thermometer in cooler; glasses stored wet; single-serve cup used as a scoop for marinara sauce; peeling skillets and heavily pitted pot; insulation exposed in cooler door; thermometer broken on dish machine; dish machine not reaching correct temperature; soiled hood filters/wire rack in keg cooler has debris and rust buildup/debris buildup on rim of dish machine door; leaking hand sink faucet; and mop stored in dirty mop water.
■ Kid’s Korner, 1265 Dooling Hollow Road, Festus, was inspected on March 18. No score given due to low priority.
■ LaChance Winery, 6035 Second St., Kimmswick, scored 88 on March 19. Violations: no thermometer in mini refrigerator; windows open with no screens; torn door gaskets; and grease buildup on oven exterior.
■ Lam’s Garden, 510 Bailey Road, Crystal City, scored 90 on March 18. Violations: hand sink blocked; boxes of single-serve items stored on floor; chipped and peeling shelves; shelves lined with cardboard; heavy ice buildup inside freezer; and dusty ceiling fan in employee restroom.
■ Mercy Hospital Jefferson Cafeteria, 1400 Hwy. 61, Crystal City, scored 96 on March 19. Violations: water pitcher in hand sink; and heavy ice buildup in freezer.
■ Midwest Petroleum, 3910 Jeffco Blvd., Arnold, was inspected on March 19. No score given due to low priority.
