During the week of Oct. 12, the Jefferson County Health Department inspected 36 food establishments. Scores ranged from 69 to 100. Some were routine inspections, while others were reinspections where scores are not assigned. All inspections are unannounced visits.
Facilities considered low priority are not assigned a score. A low priority facility does not prepare food.
According to the Jefferson County Health Department, facilities start with a score of 100, and points are reduced based on observed violations:
■ Six points are deducted for priority violations that directly contribute to foodborne illness risk.
■ Three points are deducted for priority foundation violations that indirectly contribute to foodborne illness risk or enable priority violations.
■ One point for core violations related to general sanitation and good retail practices.
■ Two points for repeat violations of any type.
Dan Kane, environmental supervisor for the Health Department, said the agency does not have ranges for good or bad scores but instead looks at the overall score, adding that readers need to look at the types of violations.
He said a couple of priority violations are more concerning than several core violations.
In certain cases, the Health Department returns to a food establishment to reinspect, allowing time for the facility to correct issues. According to the Health Department, reinspection is not solely based on the score an establishment gets; it’s determined by the type of violation. Priority and priority foundation violations need to be corrected within a certain amount of time, and priority violations typically are corrected on the day of the inspection before the inspector leaves.
Kane said the department rarely closes a food establishment, and most temporary closures involve a power outage, fire or sewage backup.

The inspection scores for the week of Oct. 12 are listed below. They are all routine inspections unless otherwise noted.
■ 21 Rock Grill and Bar LLC, 16 Meramec Heights, Arnold area, scored 90 on Oct. 14. Violations: several reduced oxygen packages of corned beef without a facility HACCP (Hazard Analysis Critical Control Point) plan; grill utensils stored on cloth towels/utensils and fryer baskets stored on top of vent hood; walk-in cooler missing gasket and evaporator fan; plywood was lining wall of walk-in cooler; and dusty ceiling and walls in walk-in cooler/soiled microwave, toaster, vent hood, shelf and fryers.
■ American Muscle, 4567 Hunter Lane, House Springs, was inspected on Oct. 17. No score given due to low priority.
■ Bandana’s Bar-B-Q, 1220 Big Bill Road, Arnold, scored 92 on Oct. 14. Violations: soiled can opener blade; pre-rinse sink not sealed to wall; inside of fryers and vent hood soiled with grease; and dust buildup on ceiling vents.
■ Bob Evans, 1003 Veterans Blvd., Festus, scored 82 on Oct. 16. Violations: cook did not wash hands before putting gloves on; no paper towels at hand sink; soiled microwave/buildup of powder on funnels of hot beverage dispenser; uncovered pumpkin pie; soda nozzle stored on floor; mold-like debris on ice machine drain line/spilled coffee under juice dispenser/spilled chocolate milk inside prep cooler; and soda syrup spill on floor/debris on wall/grease buildup behind fryers and stovetop on floor.
■ Buchheit, 4550 Gravois Road, House Springs, was inspected on Oct. 16. No score given due to low priority.
■ Cafe Smok N Pi, 12636 Hwy. 21, De Soto, scored 91 on Oct. 16. Violations: spray bottles without labels; bugs in light guards; torn or damaged gaskets on prep table and prep table cooler; dusty or soiled shelving; and leaking dish sink.
■ Captain D’s, 830 Jeffco Blvd., Arnold, was reinspected on Oct. 16. No score given.
■ Dairy Queen, 120 Jefferson Square, De Soto, scored 98 on Oct. 16. Violations: torn gasket on cooler; and soiled cooler/dusty wire shelving.

■ Four Lakes Restaurant and Event Center, 5925 Plantation Drive Suite A, Hillsboro, scored 82 on Oct. 16. Violations: incorrect sanitizer solution in dish machine; no date marked on cooked potatoes; single-serve cups and containers stored on floor; holes in lids; soiled outer door surfaces on coolers; dish sink faucet handle needs repair; and soiled floor drain.
■ Hardee’s, 12974 Hwy. 21, De Soto, scored 94 on Oct. 16. Violations: box of eggs stored on floor of walk-in cooler; scoops with food debris with clean utensils/soiled evaporator filter in walk-in cooler; leaking pipe routed to floor drain with funnel and plastic cup; and damaged grout/ceiling damage in restroom.
■ House Springs BP, 4685 Gravois Road, House Springs, was inspected on Oct. 16. No score given to low priority.
■ Imo’s Pizza, 133 Fiedler Lane, Fenton, scored 82 on Oct. 16.
