During the week of April 19, the Jefferson County Health Department inspected 30 food establishments. Scores ranged from 66-100. Some were routine inspections while others were reinspections where scores are not assigned. All inspections are unannounced visits.
Facilities considered low priority are not assigned a score. A low priority facility does not prepare food.
According to the Jefferson County Health Department, facilities start with a score of 100, and points are reduced based on observed violations:
■ Six points are deducted for priority violations that directly contribute to foodborne illness risk.
■ Three points are deducted for priority foundation violations that indirectly contribute to foodborne illness risk or enable priority violations.
■ One point for core violations related to general sanitation and good retail practices.
■ Two points for repeat violations of any type.
Dan Kane, environmental supervisor for the Health Department, said the agency does not have ranges for good or bad scores but instead looks at the overall score, adding that readers need to look at the types of violations.
He said a couple of priority violations are more concerning than several core violations.
In certain cases, the Health Department returns to a food establishment to reinspect, allowing time for the facility to correct issues. According to the Health Department, reinspection is not solely based on the score an establishment gets; it’s determined by the type of violation. Priority and priority foundation violations need to be corrected within a certain amount of time, and priority violations typically are corrected on the day of the inspection before the inspector leaves.
Kane said the department rarely closes a food establishment, and most temporary closures involve a power outage, fire or sewage backup.

The inspection scores for the week of April 19 are listed below. They are all routine inspections unless otherwise noted.
■ Baisch Nursing Center, 3260 Baisch Drive, De Soto, scored 94 on April 20. Violations: dead bugs in detached storage room; wiping cloth improperly stored; soiled wall spice rack; and dusty ceiling vent.
■ Cedar Hill Lutheran Church, 8600 Silver Lane, Cedar Hill, scored 98 on April 22. Violations: multiple pest corpses in windowsills and on storage closet floor; and cracks on center island.
■ Domino’s, 32 Gannon Square, Pevely, scored 96 on April 20. Violations: detached coving; and broken light switch with no light.
■ Festus Athletic Boosters Outdoor Concession, 1515 Mid-Meadow Lane, Festus, was inspected on April 21. No score given due to low priority.
■ Happy Wok, 3916 Vogel Road, Arnold, scored 95 on April 22. Violations: blocked hand sink; boxes of cooking wine stored on floor; and ice buildup in freezer.
■ Imo’s Pizza, 10906 Hwy. 21, Hillsboro, scored 81 on April 21. Violations: canned soda on prep table; manager used improper hand-washing technique; a cooler was holding above proper temperature; lasagna was cooling improperly; ice scoops stored on top of soiled ice machine; soiled fryer exhaust, door gaskets, dish rack, walk-in cooler evaporator vent guard and wire shelving, and mop sink; and dust near dish sink.
■ Karsch’s Village Market Bakery/Deli, 10724 Business 21, Hillsboro, scored 97 on April 21. Violations: no lid on drinks near bread slicer and microwave; dusty vents/soiled microwave; and small gap above faucet.
■ Kizer Academy, 1672 Old Hwy. M, Barnhart, scored 94 on April 21. Violation: incorrect sanitizer solution.
■ LaPachanga, 900 S. Truman Blvd., Crystal City, was reinspected on April 20. No score given.
■ Maci’s Prime BBQ mobile scored 100 on April 21.
■ Miyabi Sushi & Steak House, 892 Arnold Commons Drive, Arnold, was reinspected on April 22. No score given.

■ Papa John’s Pizza, 370 Festus Centre Drive, Festus, scored 92 on March 20. Violations: no proof of food safety certification; no staff with manager food safety certification; no paper towels at hand sink; and dusty hood.
■ Penn Station East Coast Subs, 844 Arnold Commons, Arnold, scored 76 on April 24. Violations: blocked hand sink; cooler holding foods at improper temperature; incorrect sanitizer solution; prep top lids in disrepair; dusty fan guard on ice machine/soiled coolers/buildup under soda machine nozzles; and leaking dish sink and hand sink faucets.
■ Pine Mountain Country, 37 Main St., Festus, scored 66 on April 22. Violations: not all employees had food safety certification; no proof of staff with manager food safety certification; incorrect sanitizer solution; dirty spatula stored in open container with clean utensils; several meats and cheeses one day past use-by date; mouse droppings on top shelf in cabinet; box of potato chips stored on floor; scoop inside container with handle in powder; boxes of single-serve cups and lids stored on floor; single-serve spoon in graham cracker bucket; wooden spoon stored improperly; food debris on pan inside steamer; dish sink cold water handle broken; and upstairs ceiling in disrepair.
■ Pizza Hut, 2090 Rock Road, De Soto, scored 90 on April 20. Violations: Pizza Hut training does not meet minimum standards for the food safety certification; bugs in light guards; evaporator box in walk-in freezer with ice buildup; mineral buildup in dishwasher; debris in fan guards/dusty wire shelving/soiled prep table/dusty grate; residue buildup on floor near back prep table/walls with dust and cobwebs; and missing ceiling register.
■ Raintree Market, 5997 Hwy. B, Hillsboro, scored 79 on April 21. Violations: unlabeled spray bottle; incorrect sanitizer solution; broken gasket; no sanitizing test strips on site; dusty hood; and dish sink faucet leaking.
